Release Forms for Ragnarok XIII
Please remember that everyone attending Ragnarok XIII must show positive proof of Age. A drivers license, state ID, or millitary ID will do fine. Everyone attending Ragnarok XIII must also fill out and sign the release forms for Ragnarok XIII.
If you are 17 years old or younger, you must be accompanied by an adult and have your parent/legal guardian sign and have notarized the release forms for Ragnarok XIII. You must have a signed and notarized release form in order to attend Ragnarok XIII.
The release forms may be found through the following link. If you are 17 years old or younger you must print out this form, fill it out, and have your parent/legal guardian sign it and have it notarized. Dont forget it!!! NO ID? NO FORM? NO RAGNAROK!
